Abstract

Background: In the high-velocity tertiary care environments of Western India, the dual challenges of clinician burnout and patient safety have reached a critical juncture. While theoretical frameworks for Human Resource Management (HRM) exist, there is a paucity of primary data regarding their efficacy within the unique socio-technical landscape of Mumbai and Pune's healthcare hubs.

Objective: This study aimed to analyze the impact of strategic HRM practices on clinician well-being and patient safety outcomes, specifically evaluating how structural, cultural, and digital HR interventions mitigate burnout across 10 tertiary care centers.

Methods: A cross-sectional primary study was conducted with a stratified sample of N=180 healthcare professionals (nurses and physicians) across five public and five private hospitals in Mumbai and Pune. Data were collected using the Maslach Burnout Inventory (MBI) and structured safety practice surveys. Statistical analysis utilized Pearson correlation and multiple linear regression to identify predictors of organizational excellence.

Results: High levels of emotional exhaustion (28.45 ± 6.12) were observed, with shift patterns identified as a primary catalyst (p < 0.001). Regression analysis highlighted Interdisciplinary Collaboration (β = -0.44, p < 0.001) and Work-Life Balance Policies (β = -0.32, p = 0.002) as the strongest predictors for reducing patient safety incidences. A significant "Reporting Gap" was identified, with only 30.6% of clinicians consistently reporting near-miss incidents a trend directly correlating with lower Psychological Safety scores (r = -0.55 for Intent to Leave). While the National Digital Health Mission (NDHM) transition was linked to increased technostress (r = +0.31), peer-led support networks significantly buffered this effect and improved clinical accuracy (r = +0.42).

Conclusion: The findings establish that organizational excellence in Western India’s healthcare sector is a direct result of a "Human-Centric" HRM pipeline. To elevate patient care, tertiary institutions must transition from administrative compliance to proactive advocacy, prioritizing psychological safety and structural flexibility as clinical necessities rather than organizational luxuries.
